there is no credible evidence suggesting the patients were infected deliberately. they didn’t have to tell them, because they knew they had it. what was hidden from them is the fact they were not being treated, while being told otherwise.
the reality of the experiment is pretty gruesome if you stick to the facts, no need to make up new ones.
